InterMedHx was founded in 2006 by a group of healthcare professionals representing over 75 years of combined healthcare experience. Our goal is to facilitate the integration of patients’ medical histories into healthcare provider systems and workflows. With the introduction of our first application, RxHx, we are able to seamlessly integrate a patient’s prescription history at time of patient registration, thus providing healthcare providers with valuable information in the diagnosis and treatment of patients.

  1. InterMedHx is proud to announce that Johnston Memorial Hospital in Abingdon, VA, a member of the Mountain State Health Alliance is utilizing our medication history.
  2. InterMedHx would like to welcome Sarasota Memorial Hospital in Sarasota Florida to the medication reconciliation user group. In the space of a couple of weeks, we have integrated our medication history into Sarasota Memorial Hospitals Eclipsys clinical systems for a completely automated workflow.

  3. We want to welcome HCA CJW Medical Centers in Richmond, VA to our family of solution users. CJW is utilizing a combination of automated medication history and our new "appliance" to reconcile patients medication history in the ED.  InterMedHx is now a certifited solutions partner of HCA.

  6. At a 150 bed community hospital in rural CT, InterMedHx RxHx located the home medication history for 88% of their ED patients.

RxHx: Automated Medication Inventory

InterMedHx, Inc. introduces RxHx, a new ground-breaking tool for automating the first step of the medication
reconciliation process:retrieving accurate home prescription medication histories.

 

The task of medication reconciliation, if performed at all, is difficult and costly. As a first step,
 hospital providers are tasked with obtaining a list of a patient's current home medications, health
supplements, and allergies at time of registration. A practitioner must verify the patient's medication
history along with the name, dosage, frequency and route of each medication. Verification can take
more than 20 minutes to perform. Patients are often unable to remember or unwilling to divulge the
names and dosages of their medications such that family members, physicians, and pharmacies must
be contacted.

 

This time-consuming process is now a JCAHO imperative. Given current personnel shortages, providers
are further challenged to meet today's medication reconciliation mandates in a cost-effective and accurate manner.
