Founded in 2006 by a group of healthcare professionals representing over 75 years of combined healthcare experience, our goal is to facilitate the integration of patients’ medical histories into healthcare provider systems and workflows. InterMedHx is the creator of RxHx, a first-of-its-kind automation application introduced to tackle the intensively manual, high-consequence challenge of listing home-based prescription medications at time of patient registration. A patient’s ‘medication inventory’ kick starts the medication reconciliation process, yet often relies on a most unreliable source – the patient. RxHx enables providers to rely on authoritative repositories of prescription medication information. RxHx triggers automatically each time a patient is registered, retrieves any available prescription, allergy, and OTC medication information, and prints a medication report that can be used to better support a practitioner’s diagnosis and treatment process. RxHx can be applied at any point of patient entry, and integrates seamlessly with existing hospital systems and patient care workflows.
