InterMedHx with our partner, InstantDx, developed and is delivering a state-of-the-art
"Medication Reconciliation" application which seamlessly consolidates and reconciles
patients home medication history with inpatient ordered medications and generates a
comprehensive discharge summary. Patient medication histories are provided electronically
via the SureScripts network and discharge prescriptions can be electronically prescribed, thus
ensuring a truly integrated and electronic start-to-finish reconciliation work flow.

This is the first of its kind solution that incorporates federal requirements to meet the upcoming
"Meaningful Use" clause for stimulus money. It further provides the ability to share the most
important part of the medical record, a patients inpatient medication information, with any
other hospital across the country that accesses the SureScripts network! Another industry first.

Our medication reconciliation application is easily integrated into any existing clinical system
workflow and was developed with the assistance of over 300 working physicians. It is in use in
multiple facilities across the country and a NY State health information exchange. The solution 
continues to evolve with the support and development of our member hospitals.
