
The RxHx system does not perform medication reconciliation in and of itself; it facilitates the gathering of home Rx medication data in order to better support a provider's medication reconciliation efforts.
Our medication histories originate from community pharmacy and payer databases and are not represented to be 100% complete or accurate. It is the responsibility of the prescriber/practitioner to validate/verify the information directly with the patient. RxHx does not replace the practitioner's responsibility to perform a complete medication assessment with a patient at registration.
RxHx is limited to retrieving data elements available via our medication content sources. This may include prescription drugs, patient allergies, OTC medications, or health supplements. RxHx cannot determine medications that a patient obtains from office samples, for cash, or from non-authorized sources.
Certain states prohibit disclosure of substance abuse, HIV, or psychiatric drugs, even to prevent ADRs. HIPAA privacy rules indicate that federal regulations supersede state requirements, however, there may still be issues of obtaining authorization for the disclosure of this information.
